AOPC is excited to announce that we will now be offering a new service: in-house personalized rehabilitative


therapy. This service provides our patients with the opportunity to see an athletic therapist prior to their clinical


appointment who, through their assessment, will provide each patient with a unique rehabilitation program


to complete at home. In addition, we will be conducting a series of outcome measures throughout each


patient’s entire episode of care to give feedback to our patients on their progress and monitor the influence


of our approach to care.

Purpose



The purpose of this project is to build a patient-centric model of care that incorporates education, patient participation and rehabilitative therapy with the goal of having a positive influence on patient success. By offering the tools and resources for patients to be involved in their care, it is our objective to increase our patients’  engagement which will, in turn, facilitate their ability to utilize the devices that we are supplying. By collecting outcome data from our patients at definitive time points in their appointment cycle we’re able to give feedback to our patients on their progress and monitor the influence of our approach to care.


Why


Over time, we have recognized an opportunity to further support our patients by offering a continuation of care post-release from primary care. Although, we have historically had great success with our patients, we have and always will strive for better than that.





Introduction



The purpose of The PRO Project (Personalized Rehabilitation Outcome) is to provide patients and their clinical team with an overview of their progress from their initial visit at AOPC, to their final visit with us. We believe that there is value added in analyzing outcome measures and comparing those values to ensure our patients have the best outcome possible. Along with these outcome measures, patients will be given a rehab program to facilitate reaching their personal goals that build on their hospital or prescribed rehabilitation program. Each patient will receive a report specifically designed for them which will allow them to quantify their progress throughout the entire episode of care.

    • By evaluating patient and practitioner goals and aligning our services to help facilitate the attainment of the collective objective 
    • By providing our patients take home rehabilitation programs in addition to their prosthetic/orthotic device

What To Expect



In addition to seeing a clinician, patients will also be seeing a clinical assistant (Certified Athletic Therapist) at their appointments. Our team will be collecting different outcome measures and assessing patients’ overall functional level. This data will be collected at various time points and will be compared throughout the episode of care to ensure we have measurable success. Based on this data, patients will receive a complimentary rehabilitation program to begin at home that will be included in their visit at AOPC. Furthermore, patients will also be provided with the contact information of a Physiotherapist at an affiliated sports medicine clinic  in Calgary. This is a fee for service that may be covered under group/private insurance. This service is beneficial for a one-on-one assessment and treatment, as well as a progression of an individualized rehabilitation program.
